Comprehensive Assessments and Ongoing Monitoring
At Aronsoncare, caring for aging parents starts with a comprehensive assessment. This is essential to developing an appropriate and successful care plan. We meet with both the individual and family members to determine their preferences and hear their concerns. Then we evaluate an individual’s strengths and weaknesses, and review key elements in her or his daily life. Our personalized assessment includes a review of such critical areas as: living arrangements, family and community support, financial support, review of appropriate legal documents, and eligibility for benefits. All future planning flows from this comprehensive assessment.
Care Planning
We address your loved one’s current care needs, and develop a plan of action.
If home care is preferred, we focus on the physical environment, family support, community resources, transportation, safety, financial factors, and selection and placement of home healthcare professionals.
Assistance with Family Dynamics
Change isn’t easy. Even in the best of situations, conflict may arise within families. Aronsoncare can help minimize friction, mediate issues, and develop strategies for moving forward. If necessary, professional resources can be brought in. Our priority is to identify and follow through based on the best interests of the care recipient.
Safety Assessments
Safety is our primary consideration in all cases. Is your loved one safe living alone? Can she or he manage their medications, finances, and environment? Are there any concerns about elder abuse? Whether your loved one is living at home, with a family member, or in a nursing home or long term care facility, they must feel and be safe. Our safety assessments are individualized, and provide a realistic approach to resolving challenges.
Long Distance Peace of Mind
When you can’t be there, we can. Whether you are out-of-state or out of the country, we’ll ensure that your loved one is being cared for appropriately. We’ll coordinate with you to make sure that you stay up-to-date on family needs.
Emotional Support
Emotional support is necessary for both the care recipient and the care providers and all family members. It’s challenging to have: unfamiliar people regularly in your home; stop driving; relinquish financial control; or move somewhere new with only a fraction of your belongings. Caregivers may be prone to depression and stress related illness and burnout. Having a caring yet clear-eyed professional to count on for emotional support can be crucial when you face these anxiety-provoking transitions.
Alzheimer’s & Dementia Resource
Alzheimer’s is not a part of normal aging; rather it is the most common form of dementia in older persons. Its course may be long and sometimes problematic. Aronsoncare offers ongoing strategies for management and problem-solving, resources for services, options for care, and emotional support.
Assistance with Entitlements
Medicare, Medicaid, Social Security, Disability, Long-term care Insurance, and Veterans benefits are entitlement programs which may be available to your loved one. Information and assistance in accessing these programs is incorporated into the care planning process, as appropriate. We can also help with the details and paperwork of applying for these benefits.
Placement
If facility care is necessary or desired, placement counseling is provided. There are many types of living communities now available. These include retirement and independent living, assisted living, family care, day care, and rehabilitation. We’ll work together to determine the appropriate level of care, and the “best fit” location for care based on the individual’s preferences and lifestyle. Care plans are developed with an awareness of available insurance benefits.
Referral to Other Community & Professional Services
With so many years of service in the health industry, we have acquired a portfolio of reputable professional resources and relationships. These will be used to access quality services and aid in the care process.
Transitional Care
When a patient leaves the hospital, their needs may be different from when they were admitted into the hospital. Their medications may have also changed. Their house may need modifications with medical equipment as well as other safety modifications. It is also possible that the patient may need some additional help from a certified home health aide. Perhaps the patient will need to stay in a rehabilitation facility. Bonnie Costabile has experience working with hospital discharge planners to evaluate the needs of the patient. Aronsoncare makes sure that everything is in place to allow for a smooth transition from the hospital to home or to rehab.
We provide the following services to assist with your hospital discharge planning:
- Meet with hospital discharge planners
- Evaluate the home care setting and arrange for necessary support services
- Arrange for medical equipment in the home
- Accompany patients home from the hospital
- Obtain new prescriptions and review medication protocol
- Help in evaluating, and assist in obtaining, additional support staff such as aides.
